Central Home Health Services of Texas is led by nurse practitioners who have spent multiple decades at the bedside. That clinical foundation shapes every decision we make, from how we staff our teams to how we design our care transitions.
We operate two dedicated service lines under one roof. Our Skilled Home Health team (nurses, therapists, and specialists) delivers Medicare-certified medical care. Our Provider Attendant Services team supports low-income patients with top-tier personal caregivers, funded through DADS waiver programs.
To make the hospital-to-home journey as smooth as possible, we coordinate with trusted partners across the care continuum: pharmacy home delivery, durable medical equipment, radiology, mobile labs, and Hospice. Patients and families should never have to navigate those connections alone.
